Patra Care is required by federal and state law to protect the privacy of your health information. This Notice explains:

  • How we may use and share your health information

  • Your rights regarding your health information

  • Our obligations to you

We must follow the privacy practices described in this Notice while it is in effect. We may update this Notice at any time, and changes will apply to all health information we maintain, including information created before the changes. Updated Notices will be available upon request and on our website.

How We May Use and Share Your Health Information

We may use or share your health information for the following purposes:

Treatment

  • To provide, coordinate, or manage your healthcare.

  • Example: Sharing information with your doctor, nurse, or pharmacy.

Payment

  • To bill and collect payment for services provided to you.

  • Example: Sending information to your insurance company for reimbursement.

Healthcare Operations

  • For administrative, quality improvement, and staff training purposes.

  • Example: Reviewing provider performance or conducting accreditation activities.

With Your Authorization

  • Any other use or disclosure not described in this Notice will require your written permission. You may revoke an authorization at any time in writing.

To Family, Friends, or Others Involved in Your Care

  • If you agree, we may share relevant health information to help with your care or payment for care.

  • If you are unable to agree (e.g., emergency), we may share limited information in your best interest.

  • When disclosure is mandated by law, such as reporting certain injuries, diseases, or responding to a court order.

Abuse, Neglect, or Safety Concerns

  • If we suspect abuse, neglect, or domestic violence, or to prevent a serious threat to health or safety.

Public Health and National Security

  • For public health reporting, FDA purposes, military activities, national security, or law enforcement when legally permitted.

  • We may contact you to remind you about appointments via phone, voicemail, email, or mail.

Your Rights

You have the right to:

  1. Access Your Records

    Request to view or get copies of your health information. Requests must be in writing. We may charge a reasonable, cost-based fee.

  2. Request a Correction (Amendment)

    Ask us to correct information you believe is inaccurate or incomplete. Requests must be in writing and explain why.

  3. Receive a List of Disclosures

    Obtain an accounting of certain disclosures made in the past six years (not including those for treatment, payment, or healthcare operations).

  4. Request Restrictions

    Ask us to limit how we use or share your information. We are not always required to agree but will honor agreed restrictions (except in emergencies).

  5. Request Confidential Communications

    Ask us to contact you in a specific way (e.g., phone, email, or alternate address). Requests must be in writing.

  6. Receive a Paper Copy of This Notice

    You may request a paper copy at any time, even if you have agreed to receive it electronically.
